Is fiber ok in a Crohn's or Colitis flare? The answer might surprise you. 💙There is actually no reputable organization or IBD consensus study/guidelines that suggests restricting fiber in IBD as a blanket approach... in fact it's the opposite! 💙Fiber IS important to help support butyrate production (and inflammation reduction). However, it can trigger symptoms... which is why we want to get more specific with the TYPES of fibers we use and HOW we use them. 💙As dietitians who personally live with IBD and have spent over 15 years working with the Crohn's and Colitis community, we're breaking down the real science behind fiber, your gut microbiome, and why long-term restriction might actually be keeping you stuck. 🔥 What happens in your gut without enough fiber When your helpful gut bacteria don't have an adequate fuel source, they produce less butyrate — the compound that helps lower inflammation. Less butyrate means a higher chance of inflammation and active disease activity. 🦠 How low fiber affects your gut's protective lining Without enough fiber, your helpful bacteria can't grow — and some may actually begin consuming your gut's protective mucus lining as a food source. This exposes the inner gut lining and can create an environment where flares are more likely to happen. 😔 Why you might feel stuck on a limited diet Sound familiar? Do you a red flag when you see it in the IBD nutrition space? Let's talk about it.... The cures, overpromising and the stuff that just doesn't land right. We talk about it from the perspective of an IBD dietitian and IBD patient. Top reasons why low residue didn't REALLY help you.... Here's the thing - low residue is not recommended as an effective or helpful strategy long term with IBD and can impact your symptoms and risk in the long run. In fact there is no reputable organization that recommends it for IBD because we have decades of research pointing to the importance keeping color and fiber in the diet when it comes to IBD for best outcomes. Instead of taking more things out - when we work with our patients in a flare up - we focus on modifying gut friendly foods that trigger symptoms and focus on slow advancements in the diet. No one likes doing stool tests... but you might learn to appreciate them after this video. 💩 Fecal calprotectin can be a great indicator of how things are going in the gut short of doing a colonoscopy and if you want to be proactive - you can even use the test to "predict" a flare and get ahead of it before it gets bad! 💩 A fecal calprotectin test can act as a warning signal for flares up to 8 to 12 weeks in advance. 💩 Fecal calprotectin is a protein released into the stool when white blood cells enter the intestinal wall - A rising or persistently high level indicates active mucosal inflammation, even if you feel completely fine. 💩 We use fecal calprotectin in our practice to understand better as to what's going on in the gut and if your symptoms are being driven by inflammation or something else. 💡It can be very helpful for both your GI and dietitian in terms of understanding what your next treatment steps should be! We run this marker in our practice for patients and also do in depth nutrient panels to get to the bottom of what's happening and formulate a more effective nutrition treatment path. You might not think of GLP1's would be apart of the conversation when it comes to IBD, but we continue to see more and more people trialing them for secondary conditions with IBD. In practice we have seen people often not inform their providers about GLP1 use - thinking that they do not impact gut symptoms... yet they very much do! When it comes to peptides and other random supplements - when we are in a flare we are often more vulnerable to just wanting to try anything... but it's important to exercise caution and make sure what you are taking has good evidence and is safe. Make sure to talk to your GI and dietitian about what's right for you. Also, if you find your GI doesn't have time or doesn't want to discuss your nutrition with you... we get the frustration. Yet, it's also often just not a top priority or within the scope of practice for your GI session (especially in only 15-20 minutes). Quite honestly... there is so much to unpack when it comes to that topic... tacking it on as a question at the end isn't going to get you anywhere. Plus... this is why dietitians (like us) exist! We do not use the Low Fodmap as a primary way to help those (like us) that have Crohn's, Ulcerative colitis or Microscopic colitis - in fact it can actually hold people back from experiencing symptom relief and even remission. In this video we will unpack that further. 3 Things to Consider Before Considering a Low Fodmap Approach : 🥬It doesn't address inflammation - an important consideration with IBD 🫐It's often done incorrectly which leads to more restriction, more microbiome challenges and poor response to medications (in IBD) 🌈A Low fodmap approach is meant to help you identify trigger types but doesn't explore the complexity of that in IBD and doesn't explore how to build tolerance When we work with people, we like to be thinking of dietary and nutrition approaches that have evidence for helping you: ⭐Induce remission ⭐Maintain remission ⭐Reduce risk of flare ups AND when you work with us, we will help guide you through that process and give you tools for how you can reduce symptoms without the frustration of an overly restrictive diet. It can feel confusing when it comes to figure out food intolerances in IBD - but there is so much more to the conversation that isn't really talked about much online like.... ⏰ Timing - if you are getting symptoms immediately after eating a food (like bloating or racing to the bathroom) it might not actually be the food itself - but it can still be data that we can use to drive nutrition recommendations. 💪🏼 Gut Tolerance - Triggers don't always stay the same. Did you know that the more that you build up your gut microbiome - it actually gets better at fermenting fibers overtime - which means LESS symptoms overall. 🥬 Your Food Patterns Matter Too - Many people use dietary tracking to pinpoint what their triggers are which can honestly be a little misleading in IBD.
