From 1 November 2026, two Medicare changes create new opportunities for General Practices. GP Chronic Condition Management Plans and reviews can now be completed by telephone, and the new Medicare Healthy Kids Check becomes available for every child from their third birthday until the day before they turn four. Dr Sachin Patel walks through what has changed, who is eligible, what each service needs to include, and the billing traps Practices still need to avoid. The opportunity in both cases is the same: patients may never know to ask. The Practices that benefit will be the ones that identify the right patients, explain the service, book it, and follow it up. What you'll get from this episode: How telephone care plans can remove the friction that stops chronic disease reviews from happening Which eligibility and co-billing traps still cause problems with telephone care plans How some Practices have doubled or tripled chronic disease revenue by improving follow-up systems What the new Medicare Healthy Kids Check covers and how GP and nurse time can work together Which patient lists to pull first so you can act on both changes now Resources Mentioned: AJGP: A guide to the 3-year-old health check for general practice RACGP Red Book: Guidelines for preventive activities in general practice ..... General Practice is getting squeezed. Pharmacists are taking on work that used to sit inside the Practice. Single-issue telehealth services are carving off profitable consults. Medicare continues to reward access and “free” care while more of the coordination, follow-up and unpaid work lands back with GPs. It’s easy to look at all of that and feel like someone else needs to fix it. That’s the trap. Todd and Sachin make the case that the biggest risk for Practice Owners isn’t pharmacy, policy or another new competitor. It’s waiting for someone else to rescue you while the ground keeps moving. They look at what Owners can still control, including billing, planned care, patient follow-up, capacity and the way they use their own time, and why Practices that keep improving are still doing well despite the pressure around them. What you'll get from this episode: Why passivity is a bigger threat to your Practice than pharmacy, policy or new competitors How one Owner moved private billing on Level B consults from 20% to 85% in five months Why booking a patient’s next appointment before they leave is good care, not paternalism How the drama triangle keeps GPs stuck waiting for someone else to fix the problem Why dropping a half day can improve your energy and rarely costs you billings ..... If every decision, problem and loose end in your Practice still finds its way back to you, you don't really have a delegation problem. You have an Owner bottleneck. And it's easy to see how it happens. You started the Practice. Your name was originally in every box. You know how to fix almost anything, and doing it yourself often feels faster than coaching someone else through it. The problem is that the habits that make you valuable in the consult room can work against you as an Owner. Being endlessly available, stepping in to solve the problem, and feeling responsible for getting everything right keeps the Practice dependent on you. Todd and Sachin show what has to change if you want the Practice to run without everything flowing through you: protecting leadership time, building a simple meeting rhythm, coaching instead of directing, and delegating outcomes without grabbing the work back halfway through. What you'll get from this episode: Why being the helpful, capable Owner can turn you into the biggest bottleneck Why coaching your team feels slower at first but pays off faster than constantly stepping in Why protecting half a day or a full day each week for leadership can improve the Practice and rarely costs you billings The three meetings that create a simple Practice heartbeat: alignment, leadership and finance How the 1-3-1, 10-80-10 and 20-80 tools help your team take real ownership instead of handing work back to you Resources Mentioned: Buy Back Your Time by Dan Martell ..... Most Practice Owners don’t have an ideas problem. They’ve already got a list of things that would make the Practice better. The treatment room could perform better. The team could run more smoothly. There’s a premises opportunity worth exploring. Chronic disease work could be stronger. The problem is getting one of those things all the way from “we should do this” to a finished result. In this episode, you’ll hear how one Practice turned its treatment room into a $126K annualised uplift, how another grew billings after losing two GPs, and how one Owner stayed patient for two years until the right premises finally showed up. The useful bit is what sits underneath those wins: protected time, focused effort, persistence, and getting the right people involved so the Owner isn’t carrying every project alone. What You'll Get From This Episode Why most Practice improvement projects stall before the result shows up The three ingredients behind projects that actually get finished How one Practice created a $126K annualised treatment-room uplift without adding more GPs Why spreading your attention across too many priorities kills progress How to move project work off the Owner and into the team Resources Mentioned Man’s Search for Meaning by Viktor Frankl Atomic Habits by James Clear ..... General Practice is doing more work than Medicare ever sees. Results arrive after the consultation. Referrals need chasing. Reminders need resetting. Patients show up with tests ordered elsewhere, advice from multiple providers, and no clear idea who is responsible for pulling it all together. Dr Danielle McMullen has spent years advocating at the highest levels of Australian healthcare, including two years as AMA President, while continuing to work as a GP. In this episode, she joins Todd to talk about the growing gap between the work GPs are funded to do and the work they're actually carrying, why fragmented care is pushing more responsibility back onto patients and Practices, and what General Practice needs to protect as healthcare becomes more digital and consumer-driven. What You'll Get From This Episode Why so much GP work happens outside the consultation and goes unpaid How fragmented care pushes more follow-up and coordination back onto General Practice Why patients are increasingly being asked to manage their own recalls, results and next steps Where AI and digital health help, and where clinical judgement still matters Why GP ownership and clinician-led care still matter in a more commercial healthcare system Resources Mentioned: MD Calc , the clinical decision tool referenced when discussing where AI helps and where judgement still wins. ..... Some of the GPs doing the best medicine are also the ones billing the least effectively. They're managing complex patients, doing the care planning, following things up properly, and carrying the responsibility home with them. They're just not always claiming for the work they've already done. Dr Ash Van Leeuwestyn sees this constantly. She's a GP and Medicare billing educator who audits real Practice billing data to find missed income, weak workflows and the gaps GPs often don't know are there. And the biggest blocker usually isn't laziness or poor care. It's fear. Fear of getting it wrong. Fear of audit. Old billing myths passed from GP to GP. And a billing culture where everyone is doing things slightly differently. In this episode, Ash joins Sachin to show Practice Owners where the money is being missed, how to tighten billing without compromising care, and how to build a system that doesn't rely on one GP remembering the right item number at the end of a busy consult. What You'll Get From This Episode The Medicare items GPs most commonly miss, even when the work has already been done Why conscientious GPs can be the most likely to underbill How fear of audit creates bad billing habits and lost income Why better billing starts with workflow, not memorising more item numbers How to build a Practice-wide billing system your whole team can actually follow ..... Not all Practice ownership is created equal. Some Owners have bought themselves a nightmare job. Others have built a well-paid job, a real business, or something far more valuable: an asset that can run without them. Those are the four levels Todd and Sachin use to think about Practice ownership. And the interesting bit isn't just figuring out which level you're at. It's understanding the belief, behaviour or bottleneck keeping you from the next one. In this episode, they walk through each level, what it feels like to live there, and what has to change if you want more profit, more freedom and a Practice that's worth something without you in the room. What you'll get from this episode: The four levels of Practice ownership: nightmare job, well-paid job, real business and asset The belief that keeps Owners stuck at each level Why even large or multi-Owner Practices can stall in the well-paid job layer How one Owner took the same two Practices from a $3.2M valuation to a $10.3M exit The three things that can accelerate the climb: time, skill and where you put your effort Resources Mentioned How To Buy A General Practice Without Messing It Up Why GP Ownership Isn't What You Think ..... Mary Wyatt and Sean Stevens opened Vic Park Clinic with just the two of them in 2019. A few months later COVID hit, and they opened a respiratory clinic right next door anyway. It was two consult rooms and a waiting room, under a hundred square metres. Out of that tiny space, they delivered 50,000 vaccines and became the first clinic in Australia to hit that number. Vic Park kept growing around it. Seven rooms became ten. And once they had momentum, Mary and Sean did something most Practice Owners never get to: they went looking for a second Practice. It took about two years. Roughly a dozen opportunities came across their desk, four got serious enough for an offer, and Cockburn was the one they said yes to. It's grown fast, and most days now runs around five GPs. Todd sits down with both of them to work through what actually has to change between Practice one and Practice two: the site, the systems, the culture, and one belief of Sean's that flips who the real customer is. What You'll Get From This Episode Why Sean thinks the GPs are the customer, not the patients, and what that actually changes for an Owner How Mary and Sean filtered a dozen site opportunities down to the one they said yes to Why five GPs a day is the number that decides if Practice #2 is actually working The room-allocation rule that tells you whether your culture is real or just talk Why trust between co-Owners is what actually lets either of you step away ..... Most Practice Owners don't decide they need a partner, then go looking for the right one. Someone asks to buy in. And suddenly the question becomes, “Should I say yes?” That's where things can go sideways. Because taking on a partner isn't just selling equity. You're giving someone access to risk, decision-making, profit, and the future value of the business. In this episode, Todd and Sachin share the four things they look at before bringing someone into ownership, why similarity is often overrated, and how to test a potential partner before you're locked in together. What you'll get from this episode: The four things you're really giving away when you sell equity in your Practice Why a partner who thinks exactly like you can be a liability, not an advantage How to test disagreement, temperament and decision-making before you commit Why selling equity for cash can be far more expensive than borrowing money What happens when one Owner drives the growth and everyone else shares the upside Resources Mentioned Why GP Partnerships Fall Apart (And How To Fix Them) How a Junior GP Chose the Right Practice to Buy Into ..... Only 20% of GPs would consider Practice Ownership today. It used to be 45%. The RACGP asked the ones with doubts why, and 80% said the same thing: they're not sure it's viable. Mark Sujecki has been a Practice Owner for 30 years. Two multi-doctor Practices in Mount Martha, ten doctors running through the main one at any time, and he still surfs, plays tennis, and volunteers overseas most years. He built the business so it can run without him in the room. Todd sits down with Mark to get into how he actually engineered that – what he handed over, what he held onto, and why the Owners who get their time back are usually the ones who stopped trying to run everything themselves. What You'll Get From This Episode Why Mark bought in for control, not money, and what that means for choosing your own partners The two-clinics-one-culture model, and how it runs without micromanaging either site How Mark made himself less essential to the Practice without shrinking the business The team every new Owner needs before they buy in, and why "smarter than you" is the standard Why 80% of GPs think ownership isn't for them, and the one question that changes their answer ..... On 1 October 2026, surcharging card payments becomes illegal in Australia. The RBA says the change is about transparency and lower costs for consumers. The problem for GP Practices is that the underlying card fee doesn't disappear. Someone still has to pay it. And depending on how your money flows, that could be your Practice or your GPs. Todd recently taught this live to our Project X Members. One Owner thought the impact would be tiny. Once she ran the numbers, she realised it affected seven or eight of her GPs. In this episode, Todd and Sachin walk through what actually changes, where the cost can land, the maths behind the fee increases needed to break even, and how to handle the conversation with your team. What You'll Get From This Episode Why removing the surcharge doesn't remove the fee, it just changes who pays it The maths behind a 1.5% surcharge becoming a 4.3% fee rise for your Practice, or 2.3% for the GP The hidden exposure many Owners haven't checked: GPs and tenants paying service fees or rent by card Why cash, bulk billing and direct bank transfers can create problems of their own Todd's four rules for navigating the change with your team ..... When Dr Luke Ward started looking for a Practice, he wasn't only looking for somewhere to work. He was looking for somewhere worth buying into. That changed the questions he asked. Who owned it? How long had the team stayed? Was the Practice still being built from scratch, or had it already proved it could run well without chaos? Luke eventually joined Robina Town Medical Centre and EZT Medical Centre on the Gold Coast. Today, he's one of five equal Owners across the two Practices. He breaks down how he assessed the opportunity, prepared himself for ownership, and what junior GPs should look for before buying into a Practice. What You'll Get From This Episode The questions Luke asked before choosing which Practice to join Why long-term staff retention told him more about culture than a polished pitch ever could How he prepared for ownership before anyone had formally offered him a share What Practice Owners should look for before inviting the next GP into the ownership group Resources Mentioned How to Grow a Multi Million Dollar Property Portfolio by Michael Yardney Rich Dad Poor Dad by Robert Kiyosaki The Barefoot Investor by Scott Pape The Armchair Guide to Property Investing podcast with Ben Kingsley and Bryce Holdaway Australian Institute of Company Directors course ..... Helen Barnett 's path to Practice ownership was anything but conventional. After 17 years as a professional opera singer with Opera Australia, she returned to university in her late 30s, studied science, won the University Medal, and earned a place in medical school. But by then, she knew medicine wasn't the path she wanted. Preventative care was. Nutrition was. And that decision eventually led her to co-own Dubbo Health Hub , a multidisciplinary Practice in rural NSW, alongside GP Dr Heather Malham. Helen brings a rare perspective to this episode. She has been the contractor, the clinician, and the Owner, often at the same time. That means her view on allied health isn't theoretical. It comes from years of seeing where these partnerships work, where they fall apart, and what Practice Owners often miss. What you'll get from this episode: Why allied health works best when it's built into the patient journey, not bolted onto the side What allied health practitioners are really looking for when they choose a Practice, starting with being treated as an equal Why an allied health room can cost around a quarter of what it costs to run a GP room, and what that means for pricing it fairly Why values fit matters more than simply filling a gap in your service mix How to find the right practitioner by starting with your patients and the referral patterns already inside your Practice Why "here's your room" isn't onboarding, and what weak onboarding quietly costs everyone ..... Dr Tim Koh opened his first clinic just three years after qualifying as a VR GP. Not to build an empire. To teach. Nearly 20 years later, he's built six Practices, trained more than 60 registrars, and retained some team members for 25 years. In this episode, Tim shares how registrar training became a serious growth strategy for his group, and why many GP owners may be overlooking one of the best ways to build culture, strengthen recruitment, and grow a Practice for the long term. What You'll Get From This Episode Why many GP owners avoid taking on registrars, and why Tim thinks they've got the maths backwards How he recruits for cultural fit, including what he tells every registrar candidate about how long he expects them to stay How to position registrars with patients so training becomes a strength of the Practice, not something the community has to tolerate Why the most valuable thing supervisors teach isn't clinical knowledge, but critical thinking How registrars can lift standards, challenge pessimism, and become part of a stronger long-term GP team Tim's advice for anyone considering Practice Ownership or supervision: "Be brave. Nothing makes you succeed like needing to succeed." ..... Connect with Todd: LinkedIn Connect with Dr. Tim Koh: The Good GP Podcast ..... P.S. Reservoir Medical Group has been open since 1960. Four generations of patients. A fifth now starting. For 10 years the Medicare freeze made it hard to keep the doors open. Then COVID hit on top of it. When Sachin asked Lindsay Hyde what she was trying to solve when she first joined Project X, her answer was three words: "We were trying not to go under." She didn't. Six years after taking a leap of faith and moving from 9 consult rooms to 13, they're at capacity. A nurse recently couldn't find a free room to run a health assessment. Now Lindsay is in conversations with the shopping centre about knocking on the neighbours' door. What You'll Get From This Episode How a four-person management team ended the era of good ideas dying quietly in the hallway Five previous registrars returned as fellows -- the retention number that tells the whole story "If you don't do your business CPD your Practice might die" -- the reframe that cuts through every vocation argument The Roger Federer principle Lindsay borrowed from Todd and never put down How the leap from 9 to 13 rooms worked, and why 12 months later they were nearly out of room again ..... Some multi-owner Practices have partners who are comfortable with what they've built and have no interest in what it could become. They show up, collect their share, and defer every decision to the practice manager. If you're the partner who can see exactly what the Practice could become while the others collect a cheque, this episode is about how you change that. This isn't just a stuck Practice. It's a wealth event no one is turning up to collect. What You'll Get From This Episode Why going from 25% to 15% might be the best financial move you ever make as a passive senior partner, and why Todd says the math almost always works in your favour Todd's one obligation for passive senior partners who want the upside: exactly what they must commit to and exactly what they must stop doing The Planet Saturn Practice: why a business with 20-plus years in the community has more upside than a greenfield start-up, and why the junior partner showing up is a gift to everyone at the table Why you should set a hard time limit on stuck-partner conversations, and what you put on the table when nothing has moved in six months The doctor who sold her stake for $1, built a Practice worth over $3M from scratch, and says the best thing that came out of it had nothing to do with money ..... An owner at the Practice Owners Conference thought their Practice was worth $100K. Their wife had already done the planning. After the sale, they would need to work nine more years to reach their retirement goals. When Sachin and Todd unpacked the structure, the Practice should have been worth $600K to $700K. The problem was not the medicine, and it was not even the business. It was a billing structure designed in the 1980s by an accountant who has probably since retired into the same situation. The associateship model makes one thing very clear: whoever built it was thinking about keeping 100% of their personal billings, not about what the thing would be worth when the time came to hand it on. What you'll get from this episode: Why the associateship structure is engineered to produce a Practice worth nothing at exit Todd's ready reckoner: average Practices trade at 3-4x on a 6-8% margin. Project X Practices run 10-15% margin and sell closer to $1.5M on the same billings How loss aversion keeps doctors trapped in a structure they know is wrong The 18-month versus 3-year restructure roadmap and what determines which path you take What to say to your fellow owners first, and why this conversation takes six to twelve sittings, not one The doctor who sold her stake for $1, walked away, and built a Practice worth over $3M ........... Most Practice Owners have a decision they keep pushing to "later." It might be a structural change, a hard conversation, a team decision, or a piece of Owner work that never quite makes it to the top of the list. Not because it doesn't matter, but because the Practice is full, the team needs attention, and the next quiet window feels just around the corner. Sachin had one of those decisions in 2013: a simple trust structure question for his accountant about a Vanguard ETF investment. The form got put aside, ended up in the laundry, and didn't get picked up again until November 2020. That delay may have cost close to seven figures in compounding returns. The same pattern showed up again and again at the Practice Owners Conference in Sydney. Across 62 one-on-one conversations, Todd and Sachin kept hearing different versions of the same thing: "I'm not ready yet." "I've got a few things to fix first." "Once this is sorted, I'll get to it." That's the waiting game. And for Practice Owners, it can get expensive fast. What You'll Get From This Episode Why waiting for the "right time" can quietly cost more than the decision itself The three reasons Practice Owners stay stuck in the waiting game How the "train as you play" principle helps Owners make progress inside real Practice conditions Why your team may be ready for growth before you feel ready to lead it How one Practice valued at $3.2M exited for $10.3M in two years with the same team, same two sites, and no expansion What finally breaks the pattern and gets important Owner work moving ..... Alana was once gifted a "chaos coordinator" badge at her Clinic. She wore it. At the time, it felt like recognition. Looking back, she sees it differently. Because in General Practice, there is a culture of badges and mugs and shared trauma. The shit show supervisor. The captain of chaos. An identity built around how many fires you put out each day. That identity feels like belonging. But it can also become the trap that keeps Owners, Practice Managers, and teams stuck below the line. For Owners, this episode is a reminder that a Practice cannot scale if its leadership identity is built around rescuing, reacting, and absorbing every problem. In this episode, Todd sits down with Alana Flohr -- 13 years in General Practice, former Practice Manager of a large Tweed Coast Clinic -- to name the three behaviours that keep owners and PMs stuck in the drama, and the above-the-line alternatives that change everything. What you'll get from this episode: The three "below the line" roles -- victim, hero, and villain -- and how each shows up inside a real Practice Why the hero role is often the most seductive trap for Practice Managers and Owners Alana's honest reflection on owning her own hero behaviour, including the chaos coordinator badge she once accepted How to coach your team through difficult conversations, even while you're still learning to have them yourself The "shut the door" system Alana used to protect one hour each afternoon for above-the-line thinking and improvement work Todd's recommended action for this week: share the episode with your PM, create a shared language, and agree to call it out when you see it Resources mentioned: Dare to Lead by Brené Brown .....
